  • L’elastofibroma è una lesione benigna rara che tipicamente insorge sulla parete posteriore del torace al sotto la scapola.
  • L’etiopatogenesi è ancora poco chiara e la conoscenza sulla sua diagnosi e gestione deriva principalmente dalle piccole serie di casi.
  • La diagnosi definitiva è stata fatta mediante esame istologico della massa.
  • Abbiamo analizzato le caratteristiche dei pazienti (come età, sesso, mano dominante, occupazione), i sintomi ed i segni all’ esordio, gli esami preoperatori, la gestione chirurgica e le complicanze, comparando i nostri dati con la letteratura medica corrente sull’argomento.
  • Dalla nostra analisi emerge che gli esami preoperatori sono di valore limitato e quando la presentazione clinica è tipica la diagnosi può essere fatta presuntivamente.
  • L’ intervento chirurgico dovrebbe essere eseguito sotto anestesia generale, poiché l’infiltrazione anestetica locale e la sedazione sono spesso inadeguate per ridurre il dolore intraoperatorio.

  • OBJECTIVE: to analyze the effects of work on the health of nurses who work in clinical surgery departments at university hospitals in relation to physical, social and psychological suffering and pain.
  • METHODS: a quantitative transversal study was carried out between 2012 and 2013 in four institutions in a state located in the south of Brazil.
  • RESULTS: we found that physical ailments persisted and that there were connections between social and psychological pain/suffering and variable physical activities as well as connections with accidents in the work place and the option to work shifts.

  • OBJETIVO: analisar os efeitos do trabalho na saúde de enfermeiros que atuam em clínicas cirúrgicas de hospitais universitários, relacionando-os aos danos físicos, sociais e psicológicos.
  • MÉTODO: estudo quantitativo, transversal, realizado entre 2012 e 2013 em quatro instituições de um Estado da região sul do Brasil.
  • A amostra foi composta por 65 enfermeiros que responderam questões sobre os hábitos de vida e dados sociodemográficos e a Escala de Avaliação de Danos Relacionados ao Trabalho.
  • Associações foram verificadas pelo teste Qui-Quadrado e Exato de Fisher e as correlações pelo teste de Spearmann.
  • RESULTADOS: prevaleceu o adoecimento físico, encontrando associação entre os fatores Danos Sociais e Psicológicos e as variáveis prática de atividade física, acidente de trabalho e opção pelo turno de trabalho.
  • OBJETIVO: analizar los efectos del trabajo en la salud de enfermeros que actúan en clínicas quirúrgicas de hospitales universitarios, relacionándolos a daños físicos, sociales y psicológicos.
  • MÉTODO: estudio cuantitativo, transversal, realizado entre 2012 y 2013, en cuatro instituciones de un estado de la región sur de Brasil.
  • La muestra estuvo compuesta por 65 enfermeros que respondieron preguntas sobre los hábitos de vida y datos sociodemográficos; se uso la Escala de Evaluación de Daños Relacionados al Trabajo.
  • Las asociaciones fueron verificadas con el test Chi-Cuadrado y Exacto de Fisher y las correlaciones con el test de Spearmann.
  • RESULTADOS: prevaleció la enfermedad física, encontrando asociación entre los factores Daños Sociales y Psicológicos y las variables: práctica de actividades físicas, accidente de trabajo y opción por el turno de trabajo.
  • CONCLUSIÓN: el trabajo realizado por enfermeros, que actúan en clínica quirúrgica en las instituciones hospitalarias investigadas, produce efectos que perjudican a la salud.

  • The susceptibility of clinical strains isolated from patients with infection to ceftriaxone (CTRX) and injectable beta-lactam antibiotics was determined in Japanese medical institutions in 2004.
  • In addition, since there are no large differences between strains isolated in the past and those isolated in 2004 in the antibacterial activity, CTRX is considered to be a useful antibiotic in the treatment of BLNAR infections which are increased by the causative organism of infections especially in the field of pediatrics in recent years.

  • Our objective was to compare measurement of Warner-Bratzler shear force of beef longissimus within and among institutions when each institution used its own protocol and when all institutions used a standardized protocol. In Exp.
  • 1, each of five institutions (A, B, C, D, and E) received two adjacent steaks from each of 27 beef strip loins (longissimus lumborum).
  • Warner-Bratzler shear force was measured for both steaks using the procedures normally used at each institution. In Exp.
  • 2, each institution received two adjacent steaks from each of 45 strip loins.
  • Shear force values differed within and among institutions due to protocol, execution of the protocol, and instrument variation.
  • Thus, comparisons of Warner-Bratzler shear force data among institutions are currently not valid.
  • However, it is possible for institutions to obtain the same mean shear force value and have high repeatability if a standard protocol is properly executed with calibrated equipment.

  • Data collected prospectively on 3811 renal transplantations performed between June 1977 and July 1982 by the 42 member institutions of the South-Eastern Organ Procurement Foundation were analyzed to determine the influence of donor-recipient HLA-A and HLA-B matching on patient and graft outcome.
  • Multivariate Cox regression analysis that included these and six other potential confounding variables revealed a significant association (P less than 0.001) between good HLA-A and B matching and increased graft survival.
  • Our findings indicate that good HLA-A and B matching is highly dependent on a system for sharing organs among institutions, and results in decreased graft rejection, better long-term graft function, and less need for post-transplantation immunosuppression.

  • Conflicts of interest are inherent to the majority of relationships among individuals and of these with companies and institutions and, certainly, research involving human beings is no exception.
  • This manuscript will focus on the various levels where conflict of interest issues are more prone to happen, such as in the actions of pharmaceutical industries, their relation to health professionals, the participation of universities and research institutes, the ethical research committees and their members, including the possible pressures exerted on them by researchers, sponsors and even their institutions.

  • The cross-sectional demonstration analysis was created through the medical records retrospectively of 165 consecutive patients who underwent neck dissections at our institution in the past 5 years with attention to ultrasound and MRI preoperative findings, type of neck dissection, type of identification and dissection of SAN, postoperative morbidity and survival rate.
  • Durante la chirurgia del collo, ci si imbatte frequentemente nel nervo accessorio spinale (SAN) o XI nervo cranico che, pertanto, è a rischio di lesione iatrogena con conseguente “sindrome della spalla”.
  • La dissezione del collo modificata con preservazione del SAN è basata sull’intento di minimizzare le deformità funzionali causate dalla sezione dell’undicesimo nervo.
  • L’obiettivo di questo studio è quello di descrivere le varianti intraoperatorie del nervo accessorio spinale e valutare la disfunzione della spalla nel postoperatorio.
  • Lo studio osservazionale trasversale è stato creato analizzando retrospettivamente 165 pazienti consecutivi che sono stati sottoposti a dissezione del collo presso il nostro istituto negli ultimi 5 anni, ponendo particolare attenzione ai reperti preoperatori derivanti da ecografia e risonanza magnetica, al tipo di dissezione del collo, al tipo di identificazione e dissezione del SAN, ai dati postoperatori di morbilità e sopravvivenza.
  • La più sicura identificazione del SAN avviene nel triangolo posteriore del collo, dove potrebbe essere riconosciuto in quanto emerge dal margine posteriore del muscolo sternocleidomastoideo, a livello del cosiddetto punto di Erb.
  • La distanza media tra il nervo grande auricolare e il SAN è stata di circa 0,90 cm.
  • La lunghezza media del tronco nervoso dal punto di Erb fino al punto in cui esso penetra nel muscolo trapezio è stata di circa 5,1 cm, con un range da 4,8 e 5,4 cm.
  • La diversità nel decorso dal bordo posteriore dello muscolo sternocleidomastoideo attraverso il triangolo posteriore del collo è stata riscontrata in 9 casi (15%), soprattutto a livello dell’ingresso nel triangolo posteriore del collo.
  • La frequenza di lesione postoperatoria del SAN è stata del 46,7% per le dissezioni radicali del collo, del 42,5% per le dissezioni selettive, e del 25% per le dissezioni modificate.
  • L’identificazione del SAN, step fondamentale nella chirurgia del collo, è assolutamente dipendente da un corretto studio preoperatorio attraverso la diagnostica per immagini.
  • La dissezione del collo modificata ha percentuali di controllo regionale simili a quelle di operazioni più demolitive in pazienti accuratamente selezionati, e riduce significativamente il rischio di disturbi funzionali.

  • Based on an analysis of data provided by regional specialists in the Czech Republic and by regional (district) specialists in internal medicine in Slovak republic it may be said that in the majority of health institutions the number of endocrinologists does not reach even the unsatisfactory standard (i. e. 1 endocrinologist per 150 to 250,000 population), that specialized training of endocrinologists, specialists in internal medicine and other specialist is not ensured, that there are not adequate numbers of community doctors and factory medical officers, that there are not adequate capacities of specialized laboratory examinations and that adequate collaboration with other disciplines is not ensured (the position is in particular alarming as regards immunological methods and cytological processing of specimens obtained by aspiration biopsy from the thyroid gland.
  • With regard to these data the authors recommend to raise the numbers of endocrinologists so that in institutions corresponding to hospitals with policlinics type II there will be one endocrinologist per 50-100,000 population, to improve the standard and availability of auxiliary examinations, to ensure endocrinological in-patient care at the level of hospitals with policlinics type III and to create specialized centres within the framework of the research base or university hospitals.

  • An estimated 8 million children, mostly birth to approximately 6-8 years of age, live in institutions worldwide.
  • Although efforts are being made in many countries to care for children without permanent parents in family environments (e.g., domestic adoption, foster and kinship care, reunification with biological parents), it is not likely that transitions to family alternatives will be completed in all countries in the near future; thus, institutions are likely to exist for many years if not decades.
  • But institutions need not operate in the current manner; they can be modified to be substantially more family-like in structure and in the behavior of caregivers.
  • Based on the available literature and the authors' experience, this paper describes steps that can be taken to implement such changes in residential institutions for infants and young children.

  • Respondents were chiefs of the institutions (obstetricians/midwives), nurse administrators (including midwives) of the obstetrical departments, or other nurse/midwives who were well versed in the routine care of the targeted institutions.
  • RESULTS: The overall response was 255 of the 684 institutions (37%).
  • Few institutions reported perineal massage education for 'almost all cases'.
  • Few institutions used water births.
  • A majority of institutions used antiseptic solution for perineal disinfection.
  • Few institutions routinely used episiotomies for multiparas, however routine use for primiparas was slightly more in hospitals (21%) and clinics (25%).
  • Not many institutions implemented the hands and knees position for correcting fetal abnormal rotation.
  • CONCLUSIONS: This survey has provided new information about the policies instituted in three types of institutions guiding second stage labor in four metropolitan areas of Japan.
  • There existed considerable differences among institutions' practice.
  • There were also many gaps between reported policies and evidence-based Guidelines for Midwives, therefore new strategies are needed in Japan to realign institution's care policies with evidenced based guidelines.

  • Based on historical references of scientific communication, we analyzed the issues of the Boletim do Instituto de Higiene de São Paulo and of the Arquivos da Faculdade de Higiene e Saúde Pública da Universidade de São Paulo.
  • In their 48 years of publication, their goal was to disseminate the scientific production of the institution and to legitimize the theoretical debate of the field, in addition to supporting the public health intervention models, written by leading researchers of the institution and by contributors and managers in the field of public health.
  • Both the Boletim and the Arquivos were recognized as scientific communication of national reference, and have laid the foundations for the creation of the Revista de Saúde Pública, in 1967.
  • RESUMO Com base em referenciais históricos da comunicação científica, foram analisadas as edições do Boletim do Instituto de Higiene de São Paulo e dos Arquivos da Faculdade de Higiene e Saúde Pública da Universidade de São Paulo.
  • Publicadas respectivamente de 1919 a 1946 e de 1947 a 1966, totalizam 120 edições.
  • Em seus 48 anos de publicação, o objetivo era disseminar a produção científica da instituição e legitimar o debate teórico do campo, além de apoiar os modelos de intervenção de saúde pública, escrita por importantes pesquisadores da instituição e colaboradores e gestores do campo da saúde pública.
  • Tanto o Boletim como os Arquivos foram reconhecidos como comunicação científica de referência nacional e lançaram as bases para a criação da Revista de Saúde Pública, em 1967.

  • The symptom complex of what is nowadays called "hospitalism", however, could only develop when the first hospitals were founded in early Christian cultures, which means social foundations, which were not at all hospitals like today, but social institutions for the old people, for pilgrims and--only in the third place--an asylum for poor, homeless, ill people.
  • As well as in other places where large numbers of people are crowded together under poor hygienic conditions, e.g. in prisons or on overcrowded ships, in those overcrowded hospitals, which were called "Nosokomeion" in the Byzantine world and thus might well have been institutions where nursing of sick people was done, specific epidemic diseases occurred which were rarely found in individuals living in freedom or at their domiciles.
